It could be due to a narrow Pelvi-ureteric juntion(PUJ). When one drinks a lot of alcohol, it produces a large amount of urine, however, if the PUJ is narrow, the urine in not able to flow out at that fast a rate and this results in pain in the costovertebral angle (the angle between the ribs and the vertebral column/ spine in the back). If that is the case, there is not much to worry other than that your friend should avoid drinking so much alcohol.
You should try and explain to your friend that its probably nothing too serious, but it is best to be sure and that were there something serious, it would be better to seek medical help early- delaying things wont be of any help. So even if the doc finds something bad, chances are that he wil be better able to cure it, the earlier your friend goes for a check-up. Besides assure him that you will be there too and you could accompany him if he doesnt want to go alone. Several other common causes could be causing this pain eg, UTI, stones etc, but one cant be sure. So ask him to go and meet the doc ASAP. Also, try and convince him to cut down on the alcohol- if that is what is causing the pain, why do it? Perhaps with your support (and some persistent badgering), you could convince him to see a doc!
